Maintaining your Oral Health is a large factor in maintaining your teeth for life and improving your overall health. Routine preventive dental care is the best way to keep you and your mouth healthy. Our office is happy to provide uninsured patients the opportunity to have predictable annual savings on preventive dental care as well as savings on any necessary dental treatment. We call our plan Jasper Smile Club.

We offer two savings plan packages, valid for an entire year.

​

1. The Full Plan: $399 (a $576 value): For New Patients and Existing Patients who need comprehensive x-rays every 3 years. This plan includes a Panoramic X-ray, Verticals Bitewing X-ray. Plus, member benefits listed below.

2. The Recall Plan: $335 (a $438 value): For Existing Patients who have had comprehensive x-rays within the last three years. This plan includes Bitewing X-rays. Plus, member benefits listed below.

 

The savings for Jasper Smile Club represent a 15-50% discount vs regular office fees. For many patients, this annual fee is even LOWER than what they are paying in insurance premiums.

 

These savings packages are also free of insurance company rules and regulations. Patients will have no annual maximums and no pre-approval requirements from insurance companies.

 

We are also giving members a 20% discount on any necessary dental treatment during the annual membership period for procedures paid in full on date of service.

Member Benefits include:

• 2 professional dental cleanings per 12-month period

• 2 fluoride treatments per 12-month period

• Annual Bitewing X-rays or Comprehensive X-rays every 3 years, depending on plan

• 2 examinations by the dentist

• 50% discount on take-home whitening trays

• 20% discount on all NECESSARY dental treatment at our office. (Must be paid at the start of the treatment with cash, check, or credit card. **Cosmetic services and special appliances are not discounted. Any cases requiring third party financing are also not discounted.**)

Membership is non-refundable and benefits are only usable during the 12-month period after the membership is paid in full. Failure of the patient to schedule their second preventive appointment during the 12-month period does not carry over to the next year.

 

Cancellation of Jasper Smile Club after payment will result in a reversal of all discounts. Refusal of any part of the package by a patient does not entitle the patient to additional credits.

 

Since the Jasper Smile Club entitles members to a generous discount, any appointments cancelled with less than a 24-hour notice will be charged a fee of $25 for cleanings and $50 for treatments.

 

Jasper Smile Club is renewable on a yearly basis, and fees are subject to increase in future years.
